Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Duties & ResponsibilitiesThe Addiction Consult Service is a multidisciplinary service that provides acute substance use disorder (SUD) care to patients in Emergency Departments (EDs), inpatient (IP) units, and SUD walk-in clinics, throughout the hospital. The team includes medical providers with addiction training, social workers, peer counselors, and community health workers. Together they provide expertise in medications for AUD and OUD, SUD screening, brief intervention, referral to ongoing treatment (SBIRT), peer support, care coordination, and harm reduction.Within the Addiction Consult Service, the post-discharge arm provides intensive, patient centered support to high-risk patients’ post-discharge from the ED through ongoing outreach, support surrounding social determinants of health (SDOH), and assistance with linkage to treatment.Summary Of Essential Duties And Responsibilities Conducts patient screening, assessment, level of care determination, and case consultation as needed by researching and analyzing information; making recommendations for actions. Provides opioid overdose education and harm reduction supplies to patients (naloxone kit, fentanyl/xylazine test strips). Provides services to patients through proactive outreach, referrals, and facilitation of linkages to concrete services. Oversees team efforts to develop strong relationships with patients through consistent engagement post-discharge, including in person and/or telephonic check-ins, follow-ups in community locations, and hands-on system navigation support for the duration of the patient’s program participation. Keeps electronic records, maintains referral lists, prepares reports, and related paperwork in a timely fashion, ensuring patient confidentiality. Evaluates employees’ job performance in accordance with H+H guidelines. Assesses staff training needs, developing and instructing staff clinical practice trainings as well as supporting staff with continued trainings to address social determinants of health (SDOH). Provides individual and group supervision in alignment with OASAS and NYSED supervision requirements to support team members and their professional development. Ensures operational adherence to government rules and regulations; including reporting of suspected cases of child abuse or neglect to the New York Central Registry. Serves as liaison with community agencies; researches, develops and facilitates the utilization of community resources May participate in drafting reports, policy and procedural manuals, memos or other instructional guides for site administrative and/or Central Office approval. Participates in interdisciplinary Performance Improvement (PI) and/or Quality Improvement (QI) projects regarding program initiative, case management and social work practice on complex issues May provide coverage in any of the facility’s other Addiction Services as needed.
