Role Description We are seeking a compassionate and dedicated Community Social Worker to join our team in New York, NY for a full-time, on-site role. Responsibilities include providing case management services, supporting families through individualized care plans, conducting home visits, and facilitating group workshops. The Community Social Worker will collaborate with other community organizations and internal teams to connect families with necessary resources, advocate for their needs, and track their progress. This role requires strong interpersonal skills, cultural sensitivity, and the ability to work within diverse communities. Qualifications Experience in case management, family advocacy, and community outreachStrong interpersonal, communication, and active listening skillsKnowledge of social work practices, family support, and resource allocationAbility to collaborate effectively with community organizations and stakeholdersEmpathy, cultural sensitivity, and a commitment to serving diverse populationsProficiency in organizing and facilitating group workshops or training sessionsFluency in Spanish is highly preferredBachelor’s degree in Social Work, Sociology, or a related field; a Master’s degree is a plus
