If you're excited to shape the future of home-based care, here's what the role involves.Per Visit Rate:SOC- $150Reg- $85ResponsibilitiesThe Home Health Visiting Nurse provides intermittent skilled nursing services in a face-to-face capacity to include patient residence and qualifying facilities. The Home Health Visiting Nurse is responsible for communicating the patient's progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient. This candidate can also expect to provide specialized nursing care to veterans enrolled in bowel management programs with the Veterans Affairs (VA). Comprehensive training will be provided to ensure clinical competency and confidence in delivering these services.Essential Duties & Responsibilities:Under the physician's order, admits patients eligible for home care services within 24-48 hoursAssess and evaluates patient needs/problems, identifies mutually agreed upon goals with patientsReports patient status and need for other disciplines to agency Clinical Supervisor and referring physicianReports to assigned follow-up Clinician as indicatedDevelops patient care plan that specifically addresses identified patient problems, patient problems and goalsUpdates care plans on an ongoing basis; revises and resolves patient problems and goals as changes occur and/or at recertificationCompleted admission paperwork and patient care plan submitted to Clinical Supervisor per agency policy following the admission including completed and signed admission checklistProvides intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of CareProvides Skilled Nursing visits according to visit schedule and notifies agency of need to alter schedule in any wayReports significant findings to patient's physician and Clinical Supervisor as they occurSubmits completed skilled nursing visit notes; communication notes and home health aide supervisory notes per agency policy on designated days as requested by Clinical SupervisorSubmits change orders per agency policyPerforms all OASIS time point assessment per Medicare Criteria and submits recertification paperwork per agency policy and procedureMaintains open lines of communications to all members of the continuum of care teamSupervises Home Health Aide and license and documents per Medicare criteria and per agency policy and procedureActs as a preceptor in the orientation of new nursing staff as requestedAttends staff meetings, team conferences and educational in-services per agency requirementsParticipates in Process Improvement (PI) program by assisting with collection of data and serves on PI team upon requestParticipates in discharge planning process Medicare Criteria and agency policy and procedureFollows agency policies and proceduresPerforms these and all other duties as assigned by the AdministratorAble to lift 40 pounds from floor to shoulderRepetitive walking, standing, sitting, bending, and use of handsAble to drive a car 2-4 hours per dayResponsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly CareOther duties as assignedQualificationsRequired Knowledge, Skills and ExperienceCurrent unencumbered State professional Nurse LicenseMust maintain a valid driver's license and good driving recordThe ability to make sound professional clinical judgmentThe ability to assess and document patient needs and formulate individualized patient care plans to meet those needsProficient clinical skillsExcellent verbal and written communication skillsProficiency in personal computer use, including e-mail, clinical, word processing, spreadsheet and presentation softwareAdditional Wisconsin Requirements:Active CPR CertificationPreferred Knowledge, Skills and ExperienceOne year of experience as a home care professional nurse and is competent in performing home care comprehensive assessmentBe willing to drive 50 miles to see patients.Perks and Benefits (What we offer):Competitive salary and multiple health benefit optionsCareer growth and developmentMentorship and continued learning opportunitiesEngaging and mission driven workplace401(k) matchingMileage reimbursement (if eligible)
