Your recruiter will provide the most up-to-date details during the hiring process._**How you'll make an impact in this role**+ Deliver high-quality, direct nursing care to PACE participants, including acute triage, blood draws, wound care, and participation in an after-hours/weekend on-call rotation.+ Collaborate with the Interdisciplinary Team (IDT) to develop, implement, and evaluate individualized care plans, ensuring timely updates based on participant needs.+ Participate in IDT care plan and participant/family meetings, and perform assessments across care settings such as the PACE clinic, Adult Day Center, and participant residences.+ Educate participants, caregivers, and staff on health management and end-of-life care, with flexibility to travel to homes, long-term care, and assisted living facilities as needed.**What minimum requirements you'll need**Licensure / Certification / Registration:Required Credential(s):+ BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date.
