Advanced Practice Provider (Nurse Practitioner / Physician Assistant)

The Advanced Practice Provider (APRN or PA-C) is responsible for delivering high-quality primary care to patients with both acute and chronic conditions in a fast-paced clinical environment. The role focuses on providing efficient patient care, addressing preventive care gaps, and maintaining accurate documentation while supporting value-based care initiatives. The provider will work closely with Medical Assistants and clinical staff to ensure smooth patient flow, timely chart completion, and high standards of clinical performance. Key Responsibilities Provide comprehensive primary care for patients with acute and chronic conditions. Manage an average of 23–24 patients per day using structured 20-minute appointment templates. Identify and address care gaps (HEDIS) and ensure accurate HCC/RAF coding during each patient visit. Ensure patients receive appropriate preventive screenings and follow-up care. Lead and coordinate with Medical Assistants and Scribes to prepare patient charts, review care gaps, and pend orders before visits. Maintain accurate and timely EMR documentation, ensuring 95% of charts are signed and closed within 48 hours. Follow scheduling templates to maintain consistent patient flow and clinic efficiency. Deliver patient-centered care while supporting value-based care initiatives and quality metrics.
