Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered. A per diem pharmacist opportunity is available in Lorain, OH, to support a specialty retail pharmacy dedicated to serving patients with complex medication regimens. This role emphasizes in-depth patient consultation and disease state management, particularly for individuals managing chronic illnesses. Key Responsibilities: Provide expert pharmacy services focusing on complex medication therapies, supporting patients taking 8-15 medications daily. Deliver compassionate and clear communication tailored to Medicare/Medicaid and low-income patients with diverse literacy levels. Manage and counsel patients with chronic, advanced disease states such as diabetes, hypertension, and multiple sclerosis. Utilize Pioneer Rx software to maintain accurate records and support pharmacy operations. Collaborate with healthcare teams to ensure optimal patient outcomes through medication therapy management. Qualifications: Strong retail and/or specialty pharmacy experience is highly preferred. Proven ability to manage and consult on complex disease states. Excellent patient communication and consultation skills, with patience and empathy. Familiarity with patient populations requiring compassionate care, especially low literacy and vulnerable groups.
