Role Description We are seeking a dedicated Nurse Practitioner (NP) to provide high-quality care in a long-term care (LTC) and skilled nursing facility (SNF) setting. This is a full-time, on-site role located in the Westminster, MD area within the Washington DC-Baltimore region. The Nurse Practitioner will be responsible for conducting patient assessments, creating care plans, prescribing treatments, and collaborating with healthcare staff to deliver comprehensive care. The role involves engaging with patients, families, and caregivers to ensure the best care outcomes while adhering to established protocols and policies. Qualifications Proven experience in patient assessments, diagnosing, and managing acute and chronic conditionsProficiency in creating and implementing personalized care plansExcellent communication and interpersonal skills for working with patients, families, and interdisciplinary teamsValid Nurse Practitioner certification and licensure in the state of MarylandKnowledge of regulatory requirements and healthcare compliance standards in LTC/SNF careStrong organizational skills and attention to detailUnderstanding and experience with electronic health record (EHR) systemsExperience in geriatric care or a related healthcare specialty is a plus
