Our mission is to empower members' healthcare journeys by unlocking the full potential of food as medicine.Role DescriptionAs a Dietitian, you will play a crucial role in supporting patients with several health plans in the United States. Your responsibilities include optimizing care for patients through 1-on-1 or group assessments, addressing patient inquiries, facilitating patient onboarding and offboarding, supporting patients in achieving and maximizing their health outcomes, and fostering supportive relationships with them. This role cares about our patients and aims to deliver a fantastic experience.You'll need to be fluent in Spanish and English to apply for this role. We are not currently supporting Visas.Job Description:Facilitates chronic condition management programs using virtual communication tools such as Zoom or a phone.Programs include Diabetes Self-Management, Diabetes Prevention, and Cardiovascular Disease. Programs include clinical, educational, and health coaching components.Develops individualized interventions designed to improve and maximize health outcomes.Identifies and enrolls individuals who qualify for chronic condition management programs.Establishes supportive and confidential relationships with patients.Collaborates with other team patients, such as program directors, case managers, Medical Doctors, and Health Coaches, to optimize care and solve problems.Proficient with technology - Apps, Slack, Google Calendar, Calendly, ZoomConducts outreach via phone, email, and regular mail.Provides face-to-face and telephonic lifestyle and behavior-related health coaching in both individual and group settings.Applies critical thinking skills and problem-solving techniques to the condition management process.Performs other duties as assignedQualificationsRegistered Dietitian Nutritionist (RDN). You will be required to get a license in New York, Washington, and some other states if you do not already have one.Fluent in Spanish and EnglishDemonstrated competence in multi-taskingExcellent critical thinking skillsExperience leading and developing presentations. High energy personal style and aptitude for process-oriented thinkingDemonstrated passion for health and wellnessDemonstrated proficiency in the use of technology and software products
