Find out what it means to Dare to Care Differently!Schedule:Monday - Wednesday - Friday (3 day shifts) 35 hour weeklyFloat between Belleville and AltonDay shift onlyNo weekendsFixed schedule/no on-callKey Responsibilities:Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;Develops and maintains a plan of care that describes interventions to attain expected outcomes;Implements and records interventions to attain expected outcomes in EMR;Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);Maintains a positive and respectful attitude to all patients and colleagues;Occasional travel to other nearby locations; andMedication administration and documentation.Requirements:Valid RN licenseBS/AA/diploma in nursing6 months of RN experience strongly preferred3 months of hemodialysis experience strongly preferredValid drivers license and transportationAbility to travel covering multiple locationsBLS certification (American Red Cross or American Heart Association Required); andAbility to lift up to 35 pounds is requiredThis is a safety sensitive positionBenefits:Depending on your job status, you may be eligible for the following benefits:Medical, Dental, Vision, & Life Insurance401(k) packageFlexible Spending AccountsEducation AssistanceCertification & Professional Membership ReimbursementCompensation:Compensation offers are based on education, experience, skills, and geographic region.Pay ranges from $40-$55 per hour depending upon experience
