Case Manager - Parenting Time Family

Position Summary: The Case Manager is assigned responsibility for providing elements of the casework services to individuals and families involved in Rensselaer County Family Treatment Court. The Case Manager has the primary responsibility for arranging the provision of services to the individual and family; managing the implementation of the individual’s service plan; monitoring the integration of services delivered by the provider to each individual; case recording and the scheduling process of periodic reviews. The overall responsibility for maintenance of the service plan rests with the Case Manager. The Case Manager works closely with the team of the Rensselaer County Family Treatment Court Visitation Program. Primary Duties and Essential Functions: · Comply with all requirements of the New York State Office of Court Administration as it pertains to grant funding that supports the program. · Compliance with applicable laws, rules and regulations of Rensselaer County Family Treatment Court and New York State Office of Court Administration. · Assist in preparation of case presentation to Family Treatment Court Treatment Team. · Upon admission, or within 24 hours to meet or have telephone contact with the individual and family to assist in transition into the program, complete necessary paperwork, and review policy and procedures. · Arrange for discharge and aftercare services, attend meetings with individual and family, complete necessary paperwork, and review policy and procedures. · Maintain regular casework contact with individuals and discuss issues and address concerns. · Provide regular contact with parents/guardians to keep them informed of individual’s progress, arrange visitation plans, inform of relevant meetings (case conference, service plan reviews), obtain releases of information. · Provide regular ongoing contact with referral/collateral agencies (DSS, CSE, Office of Children and Family Services, Office of Court Administration) to review individual progress, inform of visitation plans, and inform of relevant meetings and emergency situations. · Arrange and manage services/visitation schedule for individual and families. · Attend supervision as scheduled, participate in-service training, attend conferences when possible or as requested. · Coordinate, monitor and record individual appointments and schedule logistics in the least disruptive manner. · Coordinate information and prepare reports for court hearings (discharges, extensions). · Schedule and record visitation arrangements. · Schedule Service Plan Reviews and Case Consultation Meetings. · Work in conjunction with treatment team members to develop and implement strategies for consistent treatment. · Maintain regular contact with Rensselaer County Treatment Court Staff to discuss individual progress and concerns. · Meet regularly with Supervisor to case conference, problem solve and share information. · Coordinate, schedule and work with transportation staff to ensure aware of schedules and aware of each individual/ family need. Required Education, Knowledge and Skills: · Bachelor’s degree in social work, psychology, rehabilitation or related human services field from an accredited program and one year of experience in working with children/adolescents and families.
