HHC/Hospice RN (Casual)
JOB REQUIREMENTS: Essential Duties and Responsibilities include thefollowing. Other duties may be assigned. Evaluates assigned home healthcare and hospice patients and identifies problems/needs including butnot limited to physical, emotional, psychological, environmental andeducational. Develops, communicates, and coordinates a goal orientedplan of care for each assigned patient in collaboration with thepatient, M.D., and family, and for Hospice, the Interdisciplinary Group.Implements plan of care, nursing interventions, patient/family teachingand other appropriate procedures as necessary to ensure high qualitynursing care of the home care/hospice patient; delegates appropriatelyand ensures documentation of interventions. Evaluates each patient\'sresponse to the care provided and modifies the plan of care based onsigned physician orders. Maintains strict confidentiality with clientinformation. Communicates effectively and timely with all members of thehealth care team; maintains up-to-date and accurate documentation ofcare provided to ensure the integration of information for use by thehealth care team. Promotes and maintains health of individual familiesthrough teaching, counseling and initiation of appropriate preventive,rehabilitative, or palliative measures. Educates patients and familiesregarding disease management, home safety, medication safety, end oflife care, plans of care and anticipated outcomes. Provides nursingservices in the homes of and facilities for critically, chronically andterminally ill patients under a medical plan of treatment establishedaccording to agency policy. Coordinates services with otherprofessionals and service providers, and for Hospice, theInterdisciplinary Group to assure a wholistic approach to patient andfamily needs. Plans, manages caseload, and provides nursing services ina specific geographic area. Teaches, demonstrates, supervises andevaluates services provided in the home by other agency staff. Preparesand submits records and reports according to agency policy. Assumesresponsibility for self development and participates in appropriatecontinuing education activities in order to maintain competence withinthe scope of own practice. Participates in developing and implementing acommunity disaster plan. Assumes administrative responsibilities asdesignated by the HHC/Hospice Supervisor. May be assigned to performon-call duty. It is unlikely an employee will perform all the dutieslisted on a regular basis, nor is the list exhaustive in the sense itcovers all the duties an employee may be required to perform. SupervisoryResponsibilities This job has no supervisory responsibilities, butcontributes to the oversight and guidance of HHC/Hospice Agency Aides,in accordance with the organization\'s policies, procedures andapplicable laws. Provides mentorship to nursing students. \*\*\*\*\*OTHER EXPERIENCE AND QUALIFICATIONS: Comprehensive knowledge of thenursing process (assessment, planning, implementation, evaluation).Comprehensive knowledge and skill in current nursing practice. Thoroughknowledge of the use of community resources. Comprehensive knowledge ofinterprofessional and interfamily relationships in the implementation ofnursing/medical care plans. Thorough knowledge of Home Health Careprinciples and Hospice philosophy.
