HHC/Hospice RN (Part-Time)

JOB REQUIREMENTS: Essential Duties and Responsibilities include thefollowing. Other duties may be assigned. Evaluates assigned home healthcare and hospice patients and identifies problems/needs including butnot limited to physical, emotional, psychological, environmental, andeducational. Develops, communicates, and coordinates a goal-orientedplan of care for each assigned patient in collaboration with thepatient, M.D., and family, and for Hospice, the Interdisciplinary Group.Implements plan of care, nursing interventions, patient/family teaching,and other appropriate procedures as necessary to ensure high qualitynursing care of the home care/hospice patient; delegates appropriatelyand ensures documentation of interventions. Evaluates each patient\'sresponse to the care provided and modifies the plan of care based onsigned physician orders. Maintains strict confidentiality with clientinformation. Communicates effectively and timely with all members of thehealth care team; maintains up-to-date and accurate documentation ofcare provided to ensure the integration of information for use by thehealth care team. Promotes and maintains health of individual familiesthrough teaching, counseling, and initiation of appropriate preventive,rehabilitative, or palliative measures. Educates patients and familiesregarding disease management, home safety, medication safety, end oflife care, plans of care, and anticipated outcomes. Provides nursingservices in the homes of and facilities for critically, chronically, andterminally ill patients under a medical plan of treatment establishedaccording to agency policy. Coordinates services with otherprofessionals and service providers, and the Hospice Interdisciplinarygroup to assure a wholistic approach to patient and family needs. Plans,manages caseload, and provides nursing services in a specific geographicarea. Teaches, demonstrates, supervises, and evaluates services providedin the home by other agency staff. Prepares and submits records andreports according to agency policy. Assumes responsibility for selfdevelopment and participates in appropriate continuing educationactivities in order to maintain competence within the scope of ownpractice. Participates in developing and implementing a communitydisaster plan. Assumes administrative responsibilities as designated bythe HHC/Hospice Supervisor. May be assigned to perform on-call duty. Itis unlikely an employee will perform all the duties listed on a regularbasis, nor is the list exhaustive in the sense it covers all the dutiesan employee may be required to perform. Supervisory Responsibilities This job hasno supervisory responsibilities but contributes to the oversight andguidance of HHC/Hospice Agency Aides and Licensed Practical Nurses, inaccordance with the organization\'s policies, procedures, and applicablelaws. Provides mentorship to nursing students. \*\*\*\*\* OTHEREXPERIENCE AND QUALIFICATIONS: Comprehensive knowledge of the nursingprocess (assessment, planning, implementation, evaluation).Comprehensive knowledge and skill in current nursing practice. Thoroughknowledge of the use of community resources. Comprehensive knowledge ofinterprofessional and interfamily relationships in the implementation ofnursing/medical care plans. Thorough knowledge of Home Health Careprinciples and Hospice philosophy.
