Join UnitedHealth Group and our family of businesses and you will use your diverse knowledge and experience to make health care work better for our patients.Primary ResponsibilitiesAssess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of careIdentify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive servicesManage the care plan throughout the continuum of care as a single point of contactCommunicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all membersAdvocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care teamExpect to spend about 80% of your time in the field visiting our members in their homes or in long-term care facilities. You'll need to be flexible, adaptable and, above all, patient in all types of situations.Required QualificationsBachelor's degree in Social Work or other related field1+ years of clinical or case management experience2+ years' experience in long-term care, home health, hospice, public health or assisted living___+ years of experience working with MS Word, Excel and OutlookAccess to reliable transportationPreferred QualificationsA background in managing populations with complex medical or behavioral needsExperience with electronic chartingExperience with arranging community resourcesPrior field based work experienceYour confidence, decisiveness and perseverance will help you positively impact our patient lives.
