We cover South Hampton Roads area including the Lake Prince retirement community.POSITION SUMMARY:Under the direction of the RN Case Manager, provides nursing services/ patient care activities and accommodation services in accordance with standards, regulatory guidelines, and licensure requirements. Follows the plan of care while providing services. Communicates with Manger changes and collaborates with patient/family member/physician or Nurse Practitioner as needed or directed.Key Responsibilities:Provide direct nursing care to home health patients in accordance with their care plans, including medication administration, assessments, wound care, and documentation.Monitor residents' conditions and promptly notify of any changes.Collaborate with the healthcare team to deliver exceptional care.Job Type: Full-TimeSetting: A full-service CCRC including Independent Living, Assisted Living, Skilled Nursing, and RehabMinimal Education:Graduate of accredited school of practical nursing.Minimal Experience:Minimum of one (1) year nursing experience.One (1) year skilled nursing facility experience preferred.Other Qualifications:Current Nursing LicenseCPR certifiedBenefits You'll LovePTO available as soon as it's accrued - no waiting period!
