A growing company with opportunities for advancement Company sponsored training, tuition reimbursement, and other learning opportunities Job Description The MDS Coordinator is responsible for the overall coordination of the facility Medicare, Resident Assessment Instrument (MDS) and care planning process. Responsibilities Assess and evaluate the quality of care being given to long-term care residents Perform the initial interview and assess new residents Develop a care plan that has long-term and short-term goals for improvement Conduct meetings with multidisciplinary treatment teams, caretakers, and families Reach out to specialists within the community to get help in achieving care plan goals Qualifications Graduate of accredited nursing program Must have a valid, unrestricted RN license in the state Previous MDS experience preferred Previous experience in Long Term preferred, but not required     We are an equal opportunity employer and prohibit discrimination/harassment without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
