Knowledge of discharge planning, patient assessment , patient family conferences, Working with physicians on safe discharge plan , working with snf's home health , hospice, LTACH, Preferred or nice to have : candidates with these skills will be considered first. Meditech experience List typical procedures performed on unit(s): Rounding on patients that meet high risk criteria for discharge planning needs, working with MD and Nurses, Arranging medication placement, transportation Best personality Fit: Person that is detailed oriented and enjoys working with a multi team approach with patients Can handle stress well, and patient focused We have multiple opportunities available nationwide.
