Designated Coordinator
Reports to: Program Manager
The Designated Coordinator leads and empowers a team of Direct Support Professionals and IHS Staff in providing person-centered services. We are seeking an organized self-starter who can lead with compassion.
A Designated Coordinator balances detail-oriented work like regulatory compliance while mentoring their team to success. This person has a voice in major changes across the agency and is encouraged to express creative and innovative ideas that will help us continue to develop, grow and expand!
Full-time non-exempt, Monday-Friday, daytime hours 9 AM – 5 PM (40 hours per week).
Designated Coordinator Duties
Assist with training, supervision, and coordination of DSP and IHS Staff
Lead a team to develop, plan, and ensure high-quality client programming
Maintain compliance with MN Statute 245D licensing requirements, including overseeing a variety of documentation about services provided and coordinating client support team meetings
Proactively identify and correct safety/environmental hazards to ensure the health and safety of clients and employees
Oversee the enforcement of all safety/health policies and procedures at the assigned location
Assist and/or complete meetings such as intake, annual, etc with clients and update client information as needed.
Assists in the planning, development, and implementation of individual program plans.
Able to problem solve as needed and/or be creative to problem solve.
Coordination and ensuring the protection from abuse and general well-being of clients.
Employee scheduling, attendance, and mileage tracking
Be available to receive calls during and outside of work hours from staff with questions or during emergencies.
Coordinates compliance meetings between clients, program manager and case manager.
Assured program participant privacy is maintained by being knowledgeable and following HIPAA privacy and security regulation.
Available for shift coverage when needed.

Designated Coordinator Qualifications
A person with the qualifying education and work experience identified under option 1, 2, 3, or 4, may act as the designated coordinator for a program licensed under Minnesota Statutes, 245D.
Option 1: A baccalaureate degree in a field related to human services and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
Option 2: An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
Option 3: A diploma in a field related to human services from an accredited postsecondary institution and three years full-time work experience providing direct care services to persons with disabilities or persons age 65 and older.
Option 4: A minimum of 50 hours of education and training related to human services and disabilities AND four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses
Requirements:
Experience providing direct care services to persons with disabilities or persons age 65 and older?AND/OR post-secondary education that totals 4 years
Ability to learn, understand and implement Chapter 245D licensing requirements
Strong written and verbal communication skills
Strong organizational skills and ability to manage multiple priorities
Critical thinking skills and ability to independently problem solve
Ability to work collaboratively with a team, leadership experience preferred
Must be able to lift and transfer clients, adhering to the physical demands of the?job?and safety requirements/policy
Ability to travel between office locations as needed
Must be able to pass a background study with the MN Department of Human Services
Compensation:
Pay: $20.00 - $23.00 per hour
