Home Health Occupational Therapist

Overview Forest Park Home Health is seeking a dedicated and compassionate Home Health Occupational Therapist to join our healthcare team. This role involves providing personalized occupational therapy services to patients in their homes, supporting their independence and improving quality of life across diverse populations, including pediatrics, geriatrics, and individuals with disabilities. The ideal candidate will possess strong clinical skills, a passion for patient care, and the ability to work independently within a multidisciplinary team. Responsibilities Conduct comprehensive assessments of patients’ functional abilities and develop individualized care plans tailored to their needs. Assist patients with Activities of Daily Living (ADLs) such as dressing, grooming, and mobility to promote independence. Implement therapeutic interventions aimed at improving motor skills, coordination, and overall functional performance. Coordinate discharge planning and collaborate with case managers, physicians, and caregivers to ensure seamless patient transitions across care settings. Work with diverse patient populations including those with autism, post-acute conditions, ICU stays, skilled nursing facility residents, outpatient clients, and children in early intervention or school settings. Provide education and training to patients and caregivers on adaptive techniques and equipment use. Maintain detailed documentation of patient progress, treatment plans, and outcomes in compliance with healthcare regulations. Participate in interdisciplinary team meetings to optimize patient care strategies. Stay current with advancements in occupational therapy practices, physiology knowledge, medical terminology, and anatomy. Requirements Valid state licensure as an Occupational Therapist. Experience working in various settings such as home health, inpatient units, outpatient clinics, skilled nursing facilities, or schools. Strong knowledge of assisting with ADLs, discharge planning, case management, and care plan development. Experience working with people with disabilities, geriatrics, or in early intervention programs is preferred. Familiarity with acute care environments including ICU experience is advantageous. Ability to apply physiology principles along with medical terminology and anatomy knowledge effectively in clinical practice. Excellent communication skills for working with patients of all ages and collaborating within multidisciplinary teams.
