This role involves providing one-on-one care to patients in their homes, focusing on improving mobility, strength, and overall functional independence.Key Responsibilities:Evaluate and treat patients in a home health environmentDevelop and implement individualized care plansManage a daily caseload of up to 6 patientsComplete accurate and timely documentation, including OASIS assessmentsCoordinate care with interdisciplinary healthcare teamsEducate patients and caregivers on treatment plans and progressRequirements:Active Oregon Physical Therapist licenseMinimum 1 year of skilled home health experience (required)OASIS experience requiredBCLS certification requiredAbility to work independently and manage time effectivelyReliable transportation required for patient visits
