Registered Nurse (RN) – Behavioral Health Easterseals PORT Health is seeking a full-time Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. In this role, you'll provide essential, community-based care to adults living with serious mental health and substance use challenges. This is not traditional facility-based nursing. It's impactful, hands-on behavioral health work serving individuals where support matters most – in homes, shelters, and community settings. As an ACT Team RN, you will work within a multidisciplinary team delivering community-based behavioral health support. You'll provide psychotropic medication management, holistic nursing assessments, care coordination, and crisis response — all aimed at helping individuals achieve stability, independence, and improved quality of life. You will develop meaningful relationships with clients, support them through real-life challenges, and ensure seamless continuity of care across community environments. You'll collaborate closely with psychiatrists, therapists, co-occurring disorder specialists, and other behavioral health professionals to deliver truly integrated care.
