Physical Therapist Location: Salem, OR Agency: Cynet Health Pay: $2,838 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/30/2026 About the Position Job Title: Physical Therapist (PT) - Home Health Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Must have reliable transportation. Must have a current driver's license and agency-required liability insurance. Must be willing to travel up to 75 miles from the home clinic. Proficiency in HomeCare HomeBase EMR is required, must have used it within the last 6 months or complete a retraining course. Description: Provide therapy services through home visits. Conduct periodic office meetings. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day. Each visit and activity, including documentation and travel, holds a different point value. Weekly productivity expectation is 30 points. Types of cases include Start of Care and Follow Ups. Weekend requirements vary per location and profession. Must accept every patient on their schedule regardless of distance from previous appointment. Complete all documentation within 24 hours of patient appointments. Failure to complete documentation may affect billing and follow-up care. There are no guaranteed hours with this contract.
