Practical Nurse -Primary Care

Primary Care Patient Aligned Care Team (PACT) Clinical Associate LPN works in an ambulatory care setting providing Primary Care Services. The LPN/LVN serves as a one of the core members of the Patient Aligned Care Team (PACT) team. The LPN/LVN works in collaboration with the Primary Care Provider (PCP), Care Manager Registered Nurse (RN) and the Clerical Associate Medical Support Assistant (MSA) to provide patient-centered care coordination and preventative care.The LPN works in the Primary Care clinic, in collaboration, with the registered nurse, medical support assistant and medical providers. Together, with the patient and their caregiver support system, they work to manage the care of medically complex adult patients across the continuum.Main responsibility is daily management of the Primary Care Provider's (PCP) scheduled appointments and clinic flow. This is accomplished through assisting PCP with preventative care and/or chronic disease management by obtaining vital signs, offering patient education, screen Veterans for a variety of preventative maintenance or chronic disease care needs, manages immunizations, injections and all other ordered medications, assists with exams/procedures during the visit such as pelvic exams, joint injection, wound care, Teleretinal, Telederm, EKG's, Phlebotomy, bladder scans to name a few.Other responsibilities includes assisting the PACT team or other interdisciplinary team members with panel management and care coordination. Such tasks include test result notification, patient education, discharge follow-up and chronic disease management. Serves as an active participant in planning, implementation and evaluation of care.Participates in daily huddles and team meetings with PACT team members. Demonstrates, in interactions, the ability to communicate effectively and professionally with both internal and external customers.Is knowledgeable of and properly initiates nurse protocols and Standard Operating Procedures (SOP) to help deliver holistic care.
