This position provides a unique opportunity to practice within a multidisciplinary care model designed to proactively manage diabetes and endocrine disorders, with a strong focus on preventing complications such as lower-extremity wounds and amputations. This position is a clinic-based setting with the ability to provide both consultative and longitudinal care, supported by care coordination teams and advanced technologies.As Upperline continues to expand its innovative approach to complex chronic care, this role offers long-term career growth, leadership development, and partnership opportunities.
