Your recruiter will provide the most up-to-date details during the hiring process.How you'll make an impact in this role* Lead the patient's clinical journey by implementing and monitoring personalized care plans that prioritize safety, recovery, and high-quality outcomes.* Coordinate a multidisciplinary team of specialists to ensure every aspect of the patient's treatment is seamless, holistic, and well-communicated.* Advocate for patient health by vigilantly monitoring conditions and communicating critical changes to the medical team for immediate, proactive interventions.* Empower patients and their families by assessing discharge needs and coordinating a clear transition plan, ensuring they have the resources required for a successful recovery at home.What minimum requirements you'll needLicensure / Certification / Registration:* BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
