This fully remote role provides telephonic and virtual clinical care during evenings, nights, weekends, and holidays.As a key clinical resource, you will support patients and facility staff by managing acute and chronic concerns, helping reduce unnecessary hospital transfers, and ensuring continuity of care outside of standard business hours. This role is ideal for a clinically confident provider who thrives in a fast-paced environment and is comfortable making independent decisions through telehealth and telephonic assessments.Key ResponsibilitiesProvide after-hours clinical support to patients and facility staff via phone and telehealth platformsAssess and manage acute, chronic, and behavioral health concerns in collaboration with physicians and interdisciplinary care teamsEvaluate acute changes in condition, falls, medication-related concerns, and other urgent clinical issuesDetermine appropriate next steps and treatment recommendations based on clinical assessment and established protocolsCoordinate care across facilities, hospitals, and outpatient providers to ensure continuity of careDocument all patient encounters accurately in the electronic medical record in compliance with regulatory standardsCollaborate with RN triage staff and ask clarifying clinical questions to guide care decisionsParticipate in ongoing training and education to maintain clinical competency and complianceSchedule & Shift InformationApproximately 24 hours per week with a consistent set scheduleWeeknight coverage: 5:00 PM – 8:00 AMWeekend coverage: Friday 5:00 PM through Monday 8:00 AMWeekend shifts are divided into 12-hour shifts (6:00 AM – 6:00 PM)Holiday coverage begins at 5:00 PM on the last business day and continues through 8:00 AM on the next business dayHoliday shifts are divided into 12-hour shiftsAll After Hours Call Team members are required to work overnights and holidaysTwo 12-hour holiday shifts per year are requiredWhat to ExpectManage on-demand after-hours telephonic and telehealth appointmentsCases range from mild concerns to more urgent clinical situationsFalls and acute condition evaluations are commonNo H&Ps requiredAverage call volume is approximately 4–5 calls per hourTypical phone calls last about 4 minutesTelehealth appointments typically last 20+ minutesRN triage support is providedRequired QualificationsMaster’s degree from an accredited Nurse Practitioner programActive, unrestricted Nurse Practitioner licenses in Iowa (IA), Missouri (MO), and Kansas (KS) required at time of hireWillingness to obtain additional state licenses as neededNational certification as an NP (ANP, FNP, GNP, or equivalent)Active DEA license and eligibility to prescribe controlled substancesMinimum 2–3 years of Nurse Practitioner experienceExperience managing acute and chronic conditions in adult or geriatric populationsAbility to work independently and manage multiple patient interactions simultaneouslyStrong clinical assessment and decision-making skillsProficiency with EMR systems and clinical documentationPreferred QualificationsAdditional active licenses in Indiana (IN), Ohio (OH), Illinois (IL), Nebraska (NE), or Wisconsin (WI)Experience in skilled nursing, long-term care, senior living, post-acute care, urgent care, ER, or chronic illness managementStrong understanding of geriatric care and acute condition evaluationExperience with advanced illness care and end-of-life discussionsExcellent communication and collaboration skillsComfortable working in a fast-paced, evolving care environmentInterview ProcessVirtual interviewsIncludes:Team introductions and role expectationsBehavioral interview questionsClinical case-based scenariosWhy Join Us?Fully remote positionConsistent part-time scheduleMeaningful impact on vulnerable patient populationsCollaborative clinical support environmentOpportunity to provide high-quality after-hours care while maintaining flexibility
