Family Home Health and Hospice - Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.THIS POSITION CAN BE EITHER PRN OR FULL-TIME.PRN: 80 TO 140 PER VISITFULL-TIME: $85,000-95,000 PER YEARThe Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait.
