Job Title: Social Work - MSW Location: Detroit, MI 48202 Duration: 3+ Months contract (Possible Extension) Shift Details: 8-hour and 10-hour shifts available No holiday requirements Weekend rotation required Shift Options: 8-hour shifts: 8:00 AM – 4:30 PM or 3:00 PM – 11:30 PM (ED support) 10-hour shifts: 3:00 PM – 1:30 AM (ED support) Job Summary: The Social Worker provides direct services to pediatric and adolescent students and their families, addressing personal, social, and environmental challenges. This role operates primarily in a home care setting and involves collaboration with students, families, and school staff to develop and implement effective care plans. Key Responsibilities: · Provide social work services using the five phases of the social work process. · Collaborate with students, families, and school staff to support student needs. · Coordinate care with school personnel to ensure quality outcomes and efficient use of resources. · Assist with social, emotional, and lifestyle-related challenges. · Provide referrals to appropriate community resources and agencies. · Communicate effectively to promote satisfaction and achieve care goals. Education: · Master’s degree in social work (MSW) from an accredited program Required Certification & Licensure: · Current Michigan Social Worker license: LMSW or LLMSW · Valid Michigan driver’s license · Reliable, insured vehicle Skills: · Knowledge of pediatric and adolescent care · Previous home care experience · Minimum 5+ years of case management experience · At least 2+ years of recent acute inpatient case management experience · Experience with discharge planning
