*Job Summary and Responsibilities*As our Home Health Aide CNA, you will provide essential personalized support and companionship to clients in their homes, enhancing their overall quality of life and fostering independence according to individualized care plans.Every day you will assist clients with activities of daily living (ADLs) such as personal care, meal preparation, medication reminders, light housekeeping, and transportation to appointments. You will meticulously document services provided and proactively communicate any changes in the client's condition to the appropriate supervisor.To be successful in this role, you will possess a compassionate and patient demeanor, excellent communication and interpersonal skills, and a strong commitment to client dignity and well-being. Your ability to adapt to varying client needs, maintain a safe environment, and adhere to care plans with attention to detail is essential. Providing personal care including:BathsBack rubsOral hygieneShampoosChanging bed linenAssisting patients with dressing and undressingSkin care to prevent breakdownAssisting the patient with toileting activities Keeping patient’s living area clean and orderly, as appropriate Planning and preparing nutritious meals. Assisting in feeding the patient, if necessary. Taking and recording oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency). Assisting in ambulation and exercise according to the plan of care.
