Job Summary and Responsibilities As our Home Health Aide CNA, you will provide essential personalized support and companionship to clients in their homes, enhancing their overall quality of life and fostering independence according to individualized care plans. Every day you will assist clients with activities of daily living (ADLs) such as personal care, meal preparation, medication reminders, light housekeeping, and transportation to appointments. You will meticulously document services provided and proactively communicate any changes in the client's condition to the appropriate supervisor. To be successful in this role, you will possess a compassionate and patient demeanor, excellent communication and interpersonal skills, and a strong commitment to client dignity and well-being. Your ability to adapt to varying client needs, maintain a safe environment, and adhere to care plans with attention to detail is essential. Providing personal care including: Baths Back rubs Oral hygiene Shampoos Changing bed linen Assisting patients with dressing and undressing Skin care to prevent breakdown Assisting the patient with toileting activitiesKeeping patient's living area clean and orderly, as appropriatePlanning and preparing nutritious meals.Assisting in feeding the patient, if necessary.Taking and recording oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).Assisting in ambulation and exercise according to the plan of care.Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency). Job Requirements Requirements Meets the training requirements in accordance with State and Federal laws At least 18 years of age Ability to read, write and verbally report clinical information to patients, representatives and caregivers, as well as to other Home Health Agency staff and document care given Possess and maintain current BLS certification ND CNA Licensed driver with automobile that is insured in accordance with organization requirements Preferred Self-directing with the ability to work with little direct supervision Empathy for the needs of the ill, injured, frail and the impaired Demonstrates tact, patience and good personal hygiene
