Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 13 weeks Shift: Monday through Friday Hours per Shift: 8:00 am - 4:30 pm Experience: Minimum 1 year required License: Active Illinois nursing license Certifications: BLS Certification Must-Have: Experience in home health care, strong assessment skills, and good time management Description: This position involves providing home health care to patients in their residences. The average caseload consists of approximately 5 patients per day, which includes travel time. Common patient demographics include individuals with COPD, heart failure, diabetes, wounds, oncology cases, and infections. Clinical tasks may involve wound care, administration of IV antibiotics, and managing PICC lines and wound vacs. It is important to have experience with electronic medical records (EMR) systems, preferably EPIC, and be capable of performing lab draws. The nurse will be responsible for patient documentation and effective communication with patients and their families. Teamwork and collaboration with other healthcare providers are essential components of this role. A leadership person is available during business hours, with support available 24/7. Weekend work is required every fourth weekend, along with some holiday shifts as needed. The dress code is professional attire in any color.
