Whether you're just starting your career or looking to grow in a new direction, you'll find opportunities to thrive, lead, and make a meaningful impact here.Duties & Responsibilities:Assist physicians and health care team members in understanding the significant social and emotional factors related to the health problemsParticipate in the development of the plan of treatment, prepares clinical and progress notes, works with the family, utilizes appropriate community resources.Participate in discharge planning and in-service programs and acts as a consultant to other agency personnel.Qualifications:Bachelor's Degree in Social Work or related healthcare field required.Knowledgeable regarding community resources required.Excellent communication skills and ability to work in a collaborative team approach are required.SC License Bachelor of Social Work (LBSW) required.Licensed by the S.C.
