Whether you're just starting your career or looking to grow in a new direction, you'll find opportunities to thrive, lead, and make a meaningful impact here.Assists physician and health care team members in understanding the significant social and emotional factors related to the health problems; participates in the development of the plan of treatment; prepares clinical and progress notes; works with the family; utilizes appropriate community resources; and participates in discharge planning and in-service programs and acts as a consultant to other agency personnel.Duties & ResponsibilitiesEvaluates the social and emotional needs of patients related to health problems.Participates in the development of the plan of treatment.Assist patients and their families with personal and environmental difficulties which interfere with obtaining maximum benefits from medical care.Participates in discharge planning and in-service programs.Maintains expected productivity standards.Comply with agency policies for documentation.Participates in agency functions required by regulatory and accrediting agencies and agency policies.QualificationsMaster's Degree from a school of social work accredited by the Council on Social Work Education and licensed by the S.C.
