About the job Social Service Director Director of Social Services (MSW)Full-Time | On-sitePay Range: $35.00 - $50.00 per hour (based on relevant experience)Reports To: Facility AdministratorPosition OverviewWellTech partners is seeking an experienced and compassionate Director of Social Services to lead and oversee the psychosocial care of residents within our clients facility. This role plays a critical part in supporting residents and families through assessment, care planning, crisis intervention, and discharge planning, while ensuring compliance with all state and federal regulations.The ideal candidate is a strong communicator, highly organized, and knowledgeable in MDS, care planning, and interdisciplinary collaboration.Key ResponsibilitiesAssess, identify, and document residents medically related social, emotional, and psychosocial needsCoordinate care with residents, families, interdisciplinary team members, and community resourcesProvide crisis intervention and emotional support to residents and families as neededLead and manage discharge planning, ensuring smooth transitions of careEnsure accurate and timely documentation for MDS and Care PlansProvide in-service education to staff on residents social and emotional needsSupervise and support the Social Services AssistantMaintain compliance with state, federal, and corporate guidelinesBuild and maintain strong relationships with referral agencies, service providers, and community partnersAttend required staff, department head, corporate, and social services meetingsPerform additional duties as assignedQualificationsMaster of Social Work (MSW) - RequiredBachelors degree in Social Work or Gerontology - PreferredWorking knowledge of MDS, care planning, and discharge planningStrong verbal and written communication skillsAbility to work collaboratively with residents, families, and interdisciplinary teamsKnowledge of healthcare regulations and resident rightsPhysical RequirementsOccasional pushing, pulling, bending, and reachingFrequent grasping and fine motor manipulationWhy Join Us?Competitive hourly compensation based on experienceStable, full-time leadership roleOpportunity to make a meaningful impact on residents quality of lifeCollaborative and supportive work environment
