Completes assessments, monitors, coordinates, and links individuals to resources. Determines eligibility, makes referrals, and assists participants in accessing benefits and services. Works with participants to identify and reach goals. Enters client information into client tracking databases. Develops a plan of service based on assessment.The Lead Case Manager will be responsible for providing comprehensive support to older adults experiencing or at risk of abuse, neglect or exploitation. This role will ensure the safety, well-being, and independence of clients by coordinating services, advocating for their needs, and collaborating with community partners to support their overall well-being.This part-time position is funded through the Office on Violence Against Women (OVW), U.S. Department of Justice grant.
