Ambulatory Pharmacist - Northern Network
Provide comprehensive medication management for chronic disease states such as diabetes, hypertension, hyperlipidemia, asthma/COPD, osteoporosis and heart failure.About the RoleConduct patient assessments, develop care plans, and monitor therapeutic outcomes in collaboration with the healthcare team.ResponsibilitiesManage anticoagulation therapy (e.g., warfarin, DOACs) including patient education, INR monitoring, dose adjustments, and documentation.Participate in transitions of care to reduce hospital readmissions and improve continuity of care.Complete comprehensive medication reviews as required.QualificationsRequired: Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution.Completion of a PGY1 Pharmacy Practice Residency (PGY2 in ambulatory care preferred).Current pharmacist license in Michigan.Certification in Anticoagulation Management or equivalent experience.Strong clinical knowledge and communication skills.Proficiency with EHR systems and clinical documentation.Required SkillsBoard Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS).Experience working with and utilizing a collaborative practice agreement setting.Preferred SkillsOutpatient clinics and physician offices. Anticoagulation clinic setting. Occasional travel between clinic sites may be required.Location: (Whitehall/Shelby-Hart/Ludington)Working Hours: Monday–Friday, 7:30 AM–5:30 PM. Four 10-hour shifts per week. No weekends or holidays required.
