Provides social service support to include participant screening and assessments, case management, counseling and referral.Other information:● Master's degree required● LMSW or LCSW required● Two years experience in healthcare preferred● Experience working with the frail and elderly population preferred● Excellent customer service skills● Excellent interpersonal communication skillsThis job will be authorized 80.00 hours bi-weekly.
