You will:Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and EvaluationCollaborate closely with patients, families, and our interdisciplinary care team to create personalized care plansMonitor and communicate changes in patient conditions to physicians, supervisors, and colleaguesPlay a key role in the discharge planning process to support safe, successful transitionsYour skills, compassion, and critical thinking will help patients achieve greater independence, strength, and quality of life.What we offer:Fundamental to providing great care is supporting and rewarding our team.
