Evolve Healthcare is currently hiring for a CNA for our facility located in Breckenridge, MN . As a CNA you will provide 36/40 hour Day/Night rotate shifts for 13-weeks starting Asap thru
Unit Notes:
- Unit: LTC/SNF
- Shift: Day/Nights (Rotate)
-Shift notes : D/N Rotate - EOW, 6p-6a or 2p-1030p, every other holiday
- EOW first to float
- Holidays: Every other holiday
- Cerner : PCC

Requirements:
mn CNA License
BLS AHA only.
1 Year of Acute Care Experience
LTC/SNF Experience
2yr.- experience.
- # of Beds/Residents: 40
- EMR: PointClickCare
- Ratios: RNs: Days: 1:20, Nights: 1:40, CNAs: 1:10
- Common diagnosis / Types of patients: CHF, COPD, Alzheimers, Parkinsons, Diabetes
- Are there IVs or titratable drips?: Minimal IV meds
- DON/Charge Nurse/House Supervisor: Charge Nurse
- Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?): 8 or 12hr shifts - 6a-6:30p, 6p-6:30a, 6a-2:30p, 2p-10:30p, 10p-6:30a // cycle scheduling
- Weekend Requirements: Every other weekend
- Holiday Requirements: Every other holiday
- How many shifts of unit/department specific orientation?: 2-4 days
- Additional Notes: Has a home like work environment, strong team work, and strong work ethics
