If interested in this Home Health Physical Therapist position APPLY NOW for IMMEDIATE consideration Brittney Blackman | bblackman@alinestaffing.com | text 5867107970 HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION: Location: Greenville AL 36037 – 100% on-site Service Area: Butler, Crenshaw, Lowndes, and Covington Counties Payrate: $57.30/hr + $0.43/mile Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM Productivity Requirement: 30 points HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS: The Home Health Physical Therapist will perform evaluations and treatments for patients based on physician referrals. This role focuses on establishing measurable goals to help patients achieve their maximum level of independence while providing high-quality care within a home health setting. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES: Patient Evaluation: Conduct comprehensive assessments using appropriate tools and procedures; review medical history, precautions, and contraindications. Treatment Planning: Develop and implement treatment plans focused on functional improvement and independence. Clinical Oversight: Regularly re-assess the effectiveness of treatment plans and modify goals as needed based on patient progress. Collaboration: Communicate patient needs and progress to the treatment team, physicians, and family members. Supervision: Provide guidance and supervision to Physical Therapist Assistants (PTAs) and PT Aides in accordance with state practice acts. Documentation: Maintain and submit evaluation, treatment, and billing documentation within required timelines and regulatory standards. Adaptive Equipment: Recommend and assist in securing special adaptive equipment to enhance the rehabilitation program. HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS: Education: High School Diploma or GED (verifiable) Attendance: Commitment to mandatory attendance during the first 90 days. License: Active Physical Therapist license is required. Experience: Previous Home Health experience is required. Productivity: Must be able to meet a requirement of 30 points. Adherence to HIPAA, Corporate Compliance programs, and professional practice standards. Preferred Qualifications Software: Experience with HCHB (Homecare Homebase) is preferred. Take the Next Step!
