Job Title: DietitianPay Rate: $28-$36/hourSchedule: Monday-Friday, 8:00 AM - 4:30 PMWe are seeking a compassionate and detail-oriented Dietitian to support participant-centered care through comprehensive nutritional assessment, planning, and education. This role plays a key part in improving health outcomes and quality of life through individualized nutrition interventions. Physical attendance at a care facility and/or participant homes is required to perform job duties.Job SummaryThe Dietitian completes participant nutritional assessments and reassessments, evaluates nutritional status, develops and implements appropriate nutrition interventions, and provides nutrition education to participants, caregivers, and staff. The role works closely with an Interdisciplinary Team (IDT) to ensure high-quality, coordinated care.Key ResponsibilitiesParticipant Care (60%)Develop, implement, and reassess individualized Nutrition Plans of Care (POC) every six months or as participant status changesAssess participants' access to food and refer to IDT for services such as meal preparation evaluations, meal delivery programs, sack lunches, and grocery shopping supportDetermine participants' need for and order liquid nutritional supplementsDesign and support therapeutic dining programs and nutrition-related care plansProvide nutrition education to participants, caregivers, and staff; conduct home visits as neededDevelop nutrition care plans for high-risk participants, including medical nutrition therapy recommendationsCommunicate with in-home service providers regarding food availability, intake, and hydrationMonitor food intake, therapeutic diets, and dietary modifications; communicate concerns to IDTAnalyze food intake records and calorie counts; recommend changes as appropriateDocument nutrition assessments and notes in the participant medical recordCommunicate diet changes with dietary aides, participants, caregivers, and the interdisciplinary teamAssist with meal service as neededParticipate in interdisciplinary team meetings and care planning sessionsPerform additional duties as assignedAdministration (40%)Conduct kitchen sanitation and safety checks as neededMonitor lunch and snack menus for adequacy and make changes as necessaryMaintain and update sack lunch lists, supplement sheets, diet lists, and assisted dining guidesAttend care planning conferences, family meetings, and wound care meetingsAssist in training new Dietary Aides and Lead Dietary AidesParticipate in in-service training, department meetings, and work area developmentCollaborate with state food programs in compliance with applicable regulationsParticipate in Quality Assessment and Performance Improvement (QAPI) initiatives related to nutrition servicesRequired QualificationsBachelor's degree in food science, nutrition, and/or dietetics from an accredited college or universityMinimum 1 year of experience working with frail or elderly populationsKnowledge of medical terminology and ability to complete written assessments and documentationLicensure & Certification (State-Specific)FL: State Board License, Registered Dietitian (CDR), StateServ Certification within 30 days of hirePreferred QualificationsBilingual candidates preferred#KHOK
