You'll conduct routine home visits, provide wound care, administer medications, and perform health assessments-empowering patients to live healthier, safer lives in their own homes.Travel within your designated area, making meaningful connectionsWork independently with the support of an innovative organizationEnjoy a consistent schedule: 8:30 AM - 5:30 PM, 40 hours/weekCompetitive compensation and opportunities for growthResponsibilities:Perform wound care, medication management, and health assessments during home visitsEducate patients and caregivers on disease management, safety, and medication adherenceCollaborate with care teams, partner organizations, and physiciansDocument all care accurately in our state-of-the-art electronic health recordsTravel safely between patient homes, ensuring top-tier care in every visitQualifications:Active NY RN license (BSN preferred)3+ years of clinical nursing experience, including home care or similar settings1+ year working with elderly or frail populations (training provided if needed)Strong clinical assessment, critical thinking, and communication skillsAbility to travel within your assigned regionFlexibility for weekends, nights, and on-call shiftsProficiency with computers and electronic health recordsValid NY driver's license and reliable vehicle (preferred)Other Requirements:Ability to lift up to 50 pounds and navigate stairsComfortable working in diverse environmentsMedically cleared and up to date on immunizations
