This full‑time, travel‑required role focuses on provider relations, member experience, care coordination support, and facility liaison responsibilities. The Liaison conducts daily onsite visits, builds strong relationships with SNF leadership, supports prospective members and families, provides plan education, and collaborates with integrated care teams to resolve issues quickly and professionally. This position requires strong communication skills, independence, and the ability to represent ProCare Advantage across multiple Texas markets.QualificationsQualificationsMinimum 2 years of experience in a healthcare delivery setting serving elderly patients; skilled nursing facility (SNF) experience strongly preferredCNA or LVN certification preferredActive Texas health insurance license preferred, or ability to obtain licensureExperience in healthcare sales, provider relations, outreach, or market development preferredStrong interpersonal, communication, and relationship‑management skillsProficient in Microsoft Office; able to maintain accurate digital documentationReliable transportation, valid driver’s license, clean driving record, and proof of insuranceSuitable home workspace for administrative dutiesHighly organized, self‑motivated, and able to work independentlyEssential FunctionsServe as the first point of contact for facility staff, providers, members, and families; resolve inquiries within required timeframesConduct daily onsite visits to assigned SNFs to build relationships, identify needs, and escalate concerns appropriatelySupport prospective members and families by answering plan questions and coordinating handoffs to licensed Sales AgentsMaintain ongoing communication with members and families to ensure satisfaction and address concernsPartner with facility leadership and integrated care teams to support member needs, retention efforts, and plan objectivesOrganize and facilitate onsite events to promote ProCare Advantage and educate potential members about Medicare Advantage benefitsProvide ongoing education to facility staff regarding plan processes, benefits, and care coordination workflowsEngage with members considering disenrollment; identify concerns and support retention strategiesBuild and maintain strategic relationships with key providers and facilities to support network adequacy and growthAdminister quarterly member satisfaction surveys, analyze results, and coordinate follow‑up actionsDocument facility visits, member interactions, and disenrollment activities accurately and in compliance with plan standardsProvide updated enrollee rosters and ID cards to facility leadership monthlyAttend required in‑service training, continuing education, and professional development activitiesPerform other duties as assignedPlease visit cantexcc.com for more information about this location.We are an Equal Opportunity Employer.
