Staff Nurse - LPN

Overview of the Position:The Staff Nurse is an essential member of the patient center medical home team. Responsible for managing the individualized patient care, by promoting and restoring patient's health through the nursing process; collaborating with provider and multi-disciplinary team members; provides physical and psychological support.Education and Experience:Diploma in Nursing; minimum requirements or Associates in NursingLicensed by the State Board of NursingBLS requiredTwo year medical office experience preferredRequired Skills / Core Competencies:Must have the ability to collaborate with others to improve the patient experience and address needsWorks with Patients, Families and ProvidersMust possess excellent oral and written communicationMust have the ability to collaborate with others to improve quality and address needsMaintain current general medical knowledgeMust possess excellent oral and written communicationAbility to take initiative and handle various task simultaneouslyKnowledge of policies, procedures, regulations, and protocolsKnowledge of data bases (ex: EMR systems)Bilingual in English/Spanish preferredFamiliar with medication names and usageMedical terminologyAt least two years of experience in a physician's officeExcellent interpersonal skillsDetailed orientedAbility to multitaskCompassion for patientsAbility to administer a variety of treatments and medications as directedCapable of adapting to computer softwareSupervisory Responsibilities:* This position has no direct reportsEssential Job Functions:These functions will be performed by the Staff Nurse and will be the responsibility of the incumbent working with the patients, medical providers, clinical staff and non-clinical staff to ensure excellence in service for the patient.Assess the needs of patients based on data collected through history observation, physical exam, telephone triage, and analysis or diagnostic dataPerforms direct patient care: triage, assessment, procedures, treatment, patient education, and follow up instructions.Assist patients and providers in obtaining medicinal prior authorizations when requiredVerifies the patient understands the purpose of the visit.Providers preventative Care information matching the patients age, gender, and health statusMaintain knowledge of the principles of the skills needed for practical nursing to provide patient care and treatmentKnowledge of examination, diagnostic and treatment room proceduresAbility to interpret, adapt, and apply guidelines and proceduresBasic Critical Care and Chronic Illness ManagementRequired to participate in training opportunitiesHelps promote office efficiency and encounter volumesAbility to understand and administer vaccines and immunizations to pediatric and/or adult patientsResponsible for daily clinical workflow which includes but not limited to: Referral management, telephone encounters, lab results, medication refills, lab Q&A, compliance with sample medication logsWillingness to learn about AIDS Service Organizations and Community Health CentersAssist in training new nurses to the organization in the standard working procedures of HopeHealthAll other duties assignedPhysical Requirements:Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
