OverviewA rapidly growing value-based primary care organization is seeking a Family Medicine Physician to join an outpatient clinic in New Britain, Connecticut.This practice model focuses on delivering high-quality care for Medicare-aged patients through smaller patient panels, longer visit times, and strong clinical support teams.The care model is designed to prioritize quality outcomes and preventative care rather than patient volume.ScheduleMonday Friday8:00 AM 5:00 PMFull-time position40 hours per weekMinimal call (shared statewide rotation)Practice ModelOutpatient primary care clinicValue-based care environment focused on:• Preventative medicine• Chronic disease management• Population health outcomesAdditional highlights:• Patient panel capped at 450 patients per provider• 30-minute patient visitsClinical SupportEach provider works within a collaborative care team including:• Dedicated Medical Assistant• Dedicated Medical Scribe• Shared RN support (1 RN per 2 3 providers)This team-based model significantly reduces documentation burden and administrative workload.Responsibilities• Provide comprehensive primary care services for adult patients• Diagnose and manage acute and chronic medical conditions• Conduct preventative care visits and health screenings• Develop treatment plans and coordinate follow-up care• Collaborate with interdisciplinary care teams• Document patient encounters using Epic EMRRequirements• MD or DO• Board Certified or Board Eligible in Family Medicine• Active Connecticut medical license• Active DEA licensePreferred qualifications:• Experience in primary care or outpatient medicine• Experience working with Medicare or geriatric populations• Familiarity with value-based care modelsCompensationCompetitive compensation package based on experience.Salary range may vary based on factors such as experience, training, and geographic market conditions.BenefitsComprehensive benefits package including:• Medical / Dental / Vision insurance• 401(k) with employer matching• Employee stock purchase plan• Paid time off• CME allowance• Paid malpractice coverage• Wellness and employee assistance programsAdditional Information• 8-week structured orientation program• Credentialing timeline approximately 60 days
