Lutheran Social Services is seeking a Care Coordinator to support families through the Children's Long-Term Support (CLTS) Waiver Program—a federally funded Medicaid program serving children with developmental, physical, or severe emotional disabilities.As a CLTS Care Coordinator, you'll be the main point of contact for families, helping them access services that keep children safe and thriving in their home and community.
