The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.Position SummaryThe Advanced Practice Provider (Nurse Practitioner / Physician Assistant) for the Lung Transplant Program is responsible for the comprehensive management of care for a designated group of patients within the Columbia University Irving Medical Center (CUIMC) / NewYorkPresbyterian Hospital Lung Transplant Program.The APP functions as an integral member of the multidisciplinary transplant team and provides care across the continuum, including outpatient, inpatient, and peritransplant services. Responsibilities encompass clinical care delivery, care coordination, patient education, and administrative and teaching activities, in accordance with institutional policies, established clinical protocols, and the applicable collaborative or practice agreement.ResponsibilitiesCollaborates with attending physicians and multidisciplinary care team members to provide direct patient care throughout the transplant process.Obtains and documents comprehensive subjective histories related to chief complaint or reason for visit.Performs medication reconciliation and conducts focused and comprehensive physical examinations.Documents pertinent positive and negative findings using the electronic medical record.Develops a diagnostic impression and/or differential diagnosis based on subjective and objective data.Formulates and implements goaldirected treatment plans, including health maintenance, anticipatory guidance, and appropriate medical interventions, in accordance with established protocols and practice agreements.Assesses, monitors, and documents patient response to treatment and therapeutic interventions.Writes and manages prescriptions in compliance with New York State and federal regulations, including assisting with prior authorizations as required.Performs discharge education and care planning when indicated.Provides and documents patient and family education tailored to the clinical condition and developmentally appropriate health maintenance needs.Responds to patient phone calls and electronic advice requests with timely, accurate assessment, documentation, and followup.Participates in patient education seminars and transplant education initiatives.Reviews outside medical records in collaboration with physician colleagues and peers to assess the quality, safety, and appropriateness of care.Generates comprehensive transplant evaluation summary reports for presentation at multidisciplinary transplant selection meetings.Collaborates in care transitions and coordination across services and specialties.Engages in ongoing peer review, utilization review activities, competency evaluations, and Medical Boardrelated processes as required.Documents care, coding, and billing in compliance with institutional, state, and federal requirements.Participates in quality improvement initiatives and program development activities.Performs other duties as assigned to support programmatic and institutional needs.Minimum QualificationsFor Nurse Practitioners (NPs)Master's degree or higher in Nursing, or an equivalent combination of education and experience, plus one to two years of relevant clinical experienceCurrent New York State Nurse Practitioner license (or proof of application)Current New York State Registered Nurse licenseNational board certification in an appropriate NP specialtyEligibility for DEA registrationFor Physician Assistants (PAs)Bachelor's degree in Physician Assistant studies or an equivalent combination of education and experience, plus four years of relevant clinical experienceMaster's degree may substitute, in part, for required experienceCurrent New York State Physician Assistant license (or proof of application)NCCPA certificationEligibility for DEA registrationPreferred QualificationsExperience in the coordination and management of transplant patients or other complex specialty populations.Prior experience in an academic medical center or tertiary care setting.Other RequirementsEligible for credentialing and privileging at NewYorkPresbyterian Hospital.Successful completion of applicable compliance and systems training requirementsEqual Opportunity Employer / Disability / VeteranColumbia University is committed to the hiring of qualified local residents.RequiredPreferredJob IndustriesOther
