Amazing Home Care - Amazing Home Care is actively seeking a motivated and experienced Registered Nurse (RN) to perform patient visits for our Licensed Home Care Agency. Preferably, the candidate should be bilingual in English and Spanish/Creole/Cantonese/Mandarin; however, it is not a requirement.NEW GRADUATES CAN APPLY!Job Summary:Demonstrate proficient skills to admit and reassess home health patients in regards to physician ordersCoordinate and supervise HHAs in the delivery and discharge of patient careInitiate home care visits and reassess patient needs and progress on a regular basisInitiate the plan of care under doctor's ordersObserve, assess and document all patient progressNotify physician and other personnel of changes in conditions of patientsEducate patients and caregivers on disease process, medications, plan of care, and individualized treatment plan as well as in-home health careQualifications/Educational Requirements:Current NYS Registered Nurse LicenseMust be vaccinated against COVID-19Work positively and favorably with patients, families, and staffExcellent communication and time-management skillsAbility to travel to multiple patients per dayBilingual English & SpanishBenefits:4 weeks of PTO and all Legal HolidaysEmployee Contributed Health and Dental InsuranceShift: Monday to Friday, 9am to 5pm (open to flexibility)
