Partner closely with our physician leaders and care team to remotely monitor heart failure patients using medical technology. Review alerts, proactively document and interpret observations, perform heart failure medication titration and communicate with patients and clinical staff to ensure appropriate coordination of care. This position is for someone who is highly motivated, innovative and is interested in being on the forefront of building a new program.Preferred Criteria2+ years of experience in cardiovascular medicineStrong communication and interpersonal skills to instill trust and effectively collaborateRequired CriteriaBachelor's or Master's degree in Physician Assistant studiesCurrent NYS licensureNCCPA certificationJoin a healthcare system where employee engagement is at an all-time high.
