Adult Case ManagerFull-Time/Non-Exempt/HelenaJob Duties:Maintain caseload of assigned consumers, 24 billable hours weeklyComplete comprehensive strength-based assessment and reassessment every 90 annually or as neededDevelop written treatment plan that coordinates with other services including family supports and outside community services every 90 daysDevelop crisis plan based on recommendation of the clinicianMonitor progress/eligibilityHelps client obtain appropriate entitlements/advocateMay develop budget plans with client & work with contracted payee representativeMust spend 50% of time out of office in client homes or in the communityCoordinate servicesMay perform duties of ATA as requiredComplete and appropriately maintain all required paperwork per company policiesMay need to transport clients in private vehicleMinimum Qualifications:Bachelor's degree in human services field, plus as least one year of full-time experience serving people with mental illnesses (serving SDMI adults is preferred) OR individuals with other educational backgrounds who, as providers, consumers, or advocates of mental health services have developed the necessary knowledge and skills (graduation from high school or GED and a combination of relevant experience and/or education equaling at least four years.)Benefits:401K Matching ContributionsHealth InsuranceDental InsuranceFlexible Spending AccountHealth Savings AccountFlexible Work SchedulePaid HolidaysPaid BirthdayPet InsuranceWage = $17.11+ DOEAll Positions at Many Rivers Whole Health must pass Background Checks and Motor Vehicle Record Checks.About Us:Since 1976, we have been dedicated to 13 counties in Montana and seek to serve a variety of both individuals and groups in all settings.
