Title: Transition CoordinatorLocation: Pendleton, IN 46064 - OnsiteDuration: 8 monthsInterview: Phone and In-personJob Description:The Transition Specialist for Indiana's Department of Correction: Division of YouthServices provides oversight to all transition-related practices in the State's juvenilecorrectional facilities.Job Responsibilities:" Facilitate the implementation of a life skills curriculum, titled Next Steps." Utilize the Transition Coordinator Handbook in an effort to standardize thetransition practices." Evaluate the five "Transition Consultations " to be completed by every studentunder the guidance of an assigned teacher." Participate on a weekly basis in Administrative Release Committees (ARC) in eachfacility to confirm and reinforce the student's reentry plans." Do the Follow Up Phone Calls to released youth at 1, 3, 6, 9, and 12 months." The Transition Coordinators will represent the Indiana Department of CorrectionDivision of Youth Services at many conferences and meetings." The Transition Coordinators will assist in the development of a partnership withIndiana's Department of Workforce Development, HIRE, and Indiana University'sHOPE Mentoring." The Transition Coordinators will assist in the development of the individualizedEducation Learning Plan for each youth upon entry at the facility." Work with state, county and private agencies to provide re-entry servicers for youthtransitioning back into the community." Assist students with obtaining social security cards, state identification, driver'slicense, birth certificates, and additional documentation." Assist students with transitioning back to public schools" Assist students with employment, apprenticeship programs, and military serviceinformationRequired Skills:Ability to communicate effectively in a team environment.Experience with training, coaching or guiding youth.Experience collaborating and creating initiatives.Desired SkillsExperience as a spokesperson at events.Experience with building relationships and partnerships to leverage resources and services
